At the Orientation Session, you must provide:

• A completed NHS-Northshore Personal Profile Form,
• Proof of annual income - previous two years' federal tax returns with W-2 forms
• Proof of current income - three recent pay stubs
• Credit report fee

The NHS-Northshore Personal Profile Form includes a checklist that provides details on this information as well as the current credit report fees.

The information you provide to us is the same information that you would submit to a lender when you apply for a mortgage. This information will help us to assess your mortgage-readiness, develop an action plan to help you prepare for your goal of homeownership and determine your eligibility for special first time homebuyer incentives.
